Terms & Conditions of Membership
Please read these terms carefully. They govern your Life Care Supreme policy membership, monthly contributions, beneficiary entitlements, and guaranteed mortuary service delivery.
1. General Provisions & Acceptance
These Terms and Conditions constitute a legally binding agreement between you (the “Planholder” or “Member”) and Life Care Supreme OPC (“FLC”, “Company”, “we”, “us”). By signing the official Life Care Application Form, enrolling through our digital portal, or remitting premiums, you acknowledge and agree to comply with all provisions herein.
Life Care Supreme OPC operates as a duly constituted One Person Corporation under the Revised Corporation Code of the Philippines, providing comprehensive pre-need memorial care, casket manufacturing, funeral chapel accommodations, and family bereavement protection.
2. Policy Enrollment & Eligibility
Age Requirements: Applicants for standard policies (Individual, Couple, Jade, Emerald, Ruby, and Gold) must be between eighteen (18) and sixty-five (65) years old at the time of application. Senior Comfort policies accommodate individuals aged sixty (60) to eighty (80) years old.
Health Declaration: Simplified health declarations are required on the enrollment form. The Company reserves the right to verify representations during claim assessment. Accurate disclosure ensures swift, unhindered claim fulfillment.
Certificate of Membership: Upon approval of the application and validation of the initial premium payment, the Company issues an official Certificate of Life Care Membership with a unique Policy Number (e.g. FLC-XXXXXXX).
3. Contributions, Due Dates & Grace Periods
Installment Term: Standard policies are payable under a five (5) year term comprising sixty (60) monthly installments, or the equivalent in quarterly, semi-annual, or annual remittance schedules.
Spot Cash Payment: Members opting for spot cash payment upon enrollment receive a guaranteed ten percent (10%) discount against the total contract price, with full policy maturity recognized immediately upon confirmation.
Grace Period: A grace period of thirty (30) calendar days from the scheduled due date is granted for every installment. If an insured event occurs within the grace period, benefits will be delivered, deducting any unpaid installment due.
Official Receipts Required: Always demand an official Life Care Supreme printed or digital receipt for every payment made at our branch or through authorized field agents.
4. Mortuary & Funeral Service Delivery
Upon the passing of an eligible member or covered beneficiary, the Company guarantees execution of the contracted memorial service:
- Casket Provision: Standard or executive wood/metallic casket handcrafted by Life Care Casket Factory according to policy specifications.
- Embalming & Sanitary Care: Professional preservation, hygiene sanitation, and facial restoration by licensed embalmers.
- Wake Viewing: Accommodation in our air-conditioned memorial chapels (up to policy-specified days) or complete vigil equipment installation for home wake setups.
- Burial Hearse Cortege: Dedicated funeral hearse vehicle for the cortege from chapel/home to the place of worship and cemetery.
- Deceased Transfer: 24/7 first-call retrieval from residence or hospital within the designated branch radius.
5. Transferability & Reassignment Rights
100% Transferable: Any active or fully paid Life Care Supreme policy may be transferred or assigned to any living individual (family member, relative, or acquaintance) at the time of need without transfer penalty fees.
Execution: To execute a transfer, the registered planholder (or legitimate surviving heir if deceased) must present the original Certificate of Membership and an executed Assignment of Rights endorsement at any branch office.
6. Claims Notice & Required Documentation
To initiate memorial service dispatch or financial benefit disbursement, the claimant must contact our 24/7 Emergency Dispatch Hotline (0963 400 4653) and submit:
- Original Certificate of Life Care Membership or Policy ID
- Certified True Copy of the Local Civil Registrar Death Certificate
- Valid government-issued photo ID of the claimant/beneficiary
- Police report or medico-legal findings (for accidental death indemnity claims)
- Official hospital statement of account & medical summary (for hospital allowance claims)
7. Contestability & Benefit Limitations
Contestability Period: Policies are contestable within the first twelve (12) months from effectivity for natural death causes. After one year of continuous active status, the policy becomes non-contestable except for deliberate fraudulent misstatement.
Accidental Death: Covered twenty-four (24) hours after official policy validation.
Standard Exclusions: Supplemental insurance cash benefits are not payable if death arises directly from self-inflicted injuries or suicide committed within one (1) year of policy effectivity, participation in felonious criminal acts, or declared war hazards.
8. Policy Default, Lapse & Reinstatement
Failure to pay premiums beyond the 30-day grace period causes the policy to enter delinquent/lapsed status. A lapsed policy may be reinstated within two (2) years from lapse date by remitting back installments with standard administrative interest, or by re-dating the contract term upon written approval.
9. Dispute Resolution & Governing Law
These Terms are interpreted and enforced under the laws of the Republic of the Philippines. Parties agree to attempt amicable settlement through good-faith mediation. Any unresolvable legal actions shall be instituted exclusively in the competent courts of South Cotabato, Philippines.
